Use this as a language (grammar) probe, informal assessment or RTI (Response to Intervention) form to address grammatical errors in short sentences. I have made a record sheet and task cards to give them a visual with the correct answer. I evaluated using the PLS-4 and noticed that the student had a really hard time understanding this concept of which smiley face to choose when answering the question (as well as correcting the sentence), so I decided to create a tool to help him. I have included 20 sentences. You will read the sentences to the student and have them decide if they should choose the green (good sentence) smiley face or the red (bad sentence) smiley face. Some sentences are correct and some are incorrect. The incorrect sentences have task cards with visuals containing the corrected sentence. You are also getting a rubric to keep up with the student's progress during the intervention services and a student list, so that you can keep track of which students are working on this particular intervention.
