After using your preferred method of collecting data on your students’ IEP goals, this is a great way to create a graph demonstrating their progress (or lack thereof).
Within the designated week on the “Data” sheet for each goal, input the number of correct (Corr.) trials and the number of opportunities provided (Opp.) within each activity (3 activity columns are provided to demonstrate generalization, or determine if they tend to have greater success in some areas over others).
Percentages are calculated automatically as you input the data you have collected.
I generally input at the end of each week (#DataFridays).
Once you have inputted your data for the given week, the “Graph” sheets will automatically generate a data point.
I usually save one document per student, and for each goal’s “Data” sheet, I insert a text box and type in the goal (to ensure I don’t accidentally input data from another goal). Sometimes, I might also input text boxes at times to create notes, for instance, if a student is absent for a week or more, I will make a note as to why no data was taken that week.
The document is ready with dates for the 2017-2018 school year.
