Taking data in therapy groups is always a challenge. Included in this packet are two different styles of group therapy data sheets to make it a little easier!
1. Group Data Collection: This page is intended to be used multiple times, with data taken on 1in x 4in labels. One way to do this is to fill out student information, session day and time, then slide the page into a plastic sleeve. Using double-stick tape, affix the number of labels you need (still attached to their paper backing) in the blank grey boxes. Stick 1.5in x 2in sticky notes on the three boxes at the top of the page to fill out plans, upcoming IEPs, and notes for next session.
2. Multi-Group Data Collection: This page is intended to be used one time for direct data collection. Fill in students and divide the “Teacher(s) & Time” box as suits your groups’ needs (depending on your number of students per group, you may be able to fit many groups or just a few on the sheet). Fill in student goals. At this point, I’d make a few copies so you don’t have to do that again (until someone’s IEP meeting happens and their goals change, anyway!). Then use the data box to collect your notes during your group sessions!
3. Group Attendance: Use this form to keep track of a whole group’s attendance for the entire school year on one page! Dates listed in the chart are Mondays to specify the week.
