I created this since my school is strongly focusing on Guided Math this coming school year. It's basically a "simplified" lesson plan that has a section for skill being reviewed, date, students, materials needed. Then, there are 3 levels of understanding for students to "rate" themselves at the end of the Guided Math session. It also has a spot for the teacher to put her opinion.
1 - I can do this on my own without help. I understand this.
2 - I sometimes need help from my teacher. I need more practice.
3 - I need a lot of help from my teacher. I need a lot of practice.
This helps the student take responsibility, and it can help the teacher know who should stay in the group the following day vs. who has mastered the skill and can move to another group.
Personally, I have a stack of the Guided Math Data Sheets in my notebook. The first sheet that has the "ratings" listed is printed on BRIGHT paper at the front of the notebook to remind students the meaning of each number when rating themselves.
