I was feeling frustrated because I could not find a decent form to track my guided reading groups. I decided to create my own. On each page is the level of reader, and underneath the number is the corresponding letter of the level (example level one is an A, level two is a B, and so forth). Each level is listed twice on one page. There is also space to write the date, the title of book you used for that level, the number of words in the book, student names in the group, and any notes you want to make. This has helped me easily track which student belongs in which reading group. As the groups change, I can print more forms and update the groups as needed. I hope this helps you as it has helped me!
