As a teacher, I was struggling with a guided reading assessment that addressed the standards and thoroughly assessed the student's comprehension of the book. I found this useful for all guided reading levels. Students can answer questions as they go through their book, or use the pre-read/post-read recommendations. Because we were not allowed to check the books out to students, I made a column for dates/page numbers to mark individual student's places in the book.
