I use this document during small reading groups to collect data. It is great for students in special education to record progress monitoring data, as well as for intervention data. The decoding column is used for words that they missed so that I can work those skills into later phonics lessons, comprehension is for accuracy in literal and inferential comprehension questions that they answer as we read, fluency is for their quick checks for words per minute and notes for expression and stopping at periods, skipping words/lines, etc., and writing is for notes regarding their written responses, spelling issues, etc. Email me if you would like more or fewer spaces on the data sheet to accommodate the number of students in your group. There is also a section at the bottom for lesson reflection, which is a large part of TPGES.
