I created this Guided Reading Lesson Plan after trying to use other formats that just didn't work for me. I found lots of great ideas from some fabulous Guided Reading professionals (Jan Richardson & Linda Dorn & Carla Soffos) and created this design based on my own needs. This is a 2-page format to be copied front and back. The first day, or phase 1, includes space to write my introduction to the book, the goal or focus for the lesson, a place to write down new tricky words, and a review for sight words. The first page also includes prompts to use for discussion after the first read to increase comprehension, fluency, and decoding strategies.
On the back page I have written out a short plan for day 2, or phase 2. I've included space for you to jot down which students you hope to do a Running Record on, and space for a writing prompt, and a large space for any other comments. I've dressed it up a bit with some fun little clip art pictures and a nice visually appealing font for my own enjoyment.
