Here is the lesson plan format I use for my guided reading groups. It incorporates the Common Core strategies of wide and close reading as well as text complexity. It is broken down to be used with 4 reading groups, but you can edit depending on the number of groups.
Here is a breakdown of some particular parts of the plan.
-'Intensive, Strategic, Benchmark, Advanced': These are the group levels given in our curriculum, Lead 21; You could edit with the names for your groups.
-'Bumblebees, Spiders, Ladybugs, Caterpillars': I preferred to call my reading groups by these bug names rather than, "Intensive group, your turn!"; You could edit these with the names you call your groups
-'Text Complexity': Common Core standards put a large emphasis on text complexity; Read more here-http://www.corestandards.org/assets/Appendix_A.pdf
Please feel free to edit as needed. It worked great for me last year with my kindergarten groups.
