This recording sheet is for making quick notes about your observations of students during your guided reading sessions.
Have you ever noticed something important about a student during a lesson, told yourself to write it down afterwards, and then forgotten what it was? I have!
I devised this form to allow me to jot down notes about individual readers without having to waste time writing terms such as vowels
You can choose from two forms. One form lists the following areas and skills:
• Phonemic Awareness
• Sight Words
(In Isolation, In Context)
There is also a blank form
, in case you want to list your own areas and skills. Suggestions are given for ways to code your observations.
