I created this Guided Reading & Assessment Toolkit to help make your life a little bit easier and help keep you organized.
For guided reading, I created different sheets for reading levels A-D. Each page has a mini cheat sheet of possible teaching points/observations for that reading level. You can simply check off the teaching points or behaviors you are trying to teach or use them to write in what each child is working on. I also included space for any tricky words or sight words that you might want to explain to the kids ahead of time.
In addition to the guided reading group sheet is a class grid with reading behaviors, concepts of print and print strategies so you can have a visual as to where all students are to help you make your reading groups. I also made a yearly grid to track student reading levels with the benchmarks on top for grades K and 1.
I also included a conferring sheet I used that can be used across all subject areas. I would simply put the date, what I taught/noticed and plugged in possible next steps. This system worked well for me because I always had student teachers or assistant teachers that could easily see what they could work on with that individual student if I was working with a small group. To stay organized, I would print them out in different colors for each subject.
Last, I included a fun behavior management tool I used for my class. It is always hard to run a small group when you have a large class, especially with the little ones! I would wear the “I am invisible” necklace so students knew that they could not get up and talk to me. My assistant or student teacher would wear the “Look at me!” necklace. Simply cut and laminate.
