Here and there I have my students fill one of these out after looking at their grades, so that they can become more responsible. The great thing about this is it allows them and their parents to see what actions they can take in order to get a better grade. Something I really believe is that most students who have bad grades, have that grade because they don't know how to get a better grade. This sheet allows students to own up to why they have a bad grade, while also allowing them to ask themselves what contributes to their success when making good grades. At the end of the document it has a section for them to circle ways to bring up their grade.

This is very effective to go over with students what actions they can take in order to get themselves prepared to make better grades. It only takes a few minutes to sit with each student and go over this. After going over it with the child, I sign it and have them sign it. Then, I have them take it home for the parents to sign. That way we are all on the same page about the student's academic progress.
