As an Itinerant Teacher of the Deaf, it is my responsibility to teach my DHH (Deaf and Hard of Hearing) Students how to maintain and monitor their hearing aids on a daily basis. In order to collect data for IEP Goals and Objectives, I created this Hearing Aid Weekly Time Card so that my student could increase their self-advocacy skills, as well as have a visual of their progress. The Time Card is formatted in a PDF File so that the teacher can print 4 weeks at a time, on one page.
*The different between this listing and the Hearing Aid Weekly Time Card is that this listing does NOT list Right and Left Ears or Yes/No Circling Options on the Time Card.
The Time Card allows the student to document the dates throughout the week, their name, whether they cleaned their hearing aids, checked the batteries and carried/maintained their hearing aid kit or case. The classroom teachers appreciated this, because it held the student accountable for their equipment and usage, as well as gave them data to show parents and/or guardians. Once completed at the end of an academic week, the classroom teacher would give it to the Teacher of the Deaf and I held onto these for data collection purposes.
