This tool has proven to be an effective classroom management tool for students requiring the use of an individual behavior plan. A sample chart is included on page 2 of the file. Determine the targeted behavior you want the student to focus on. Then list the five check-in times throughout the morning and afternoon. If the student has been successful during the given time period, he/she is able to color in the corresponding hand. If he/she earns all five consider a larger reward or recognition. I've copied these sheets back-to-back and send them home daily to correspond with parents and to allow them to reinforce the expectations and behaviors at home.
