This was a behavior tracker that I created for a student with a Behavior Intervention Plan (BIP). The tracker was aligned to desired behaviors (called targets) and her progress was noted by all of her teachers. This tracker can be used with secondary students who transition throughout the day to multiple teachers. Students are expected to carry this tracker and obtain signatures themselves. I attached two versions of the tracker. The first is a morning/afternoon tracker and the second is a daily tracker. Over time, I transitioned to the morning/afternoon tracker because my student would often lose her daily tracker. Depending on the needs of the student you may have to use the morning/afternoon tracker or even a period by period tracker. However, this depends on how your student keeps up with his or her tracker.
