This reading response sheet has sentence starters that get students writing thoughtful responses about what he/she has read. The directions require the students to use text evidence, and could be the start of practicing text dependent writing. However, the document can be edited. I use this instead of a quick reading quiz to hold students accountable because it is more rigorous. With this, you can tell the students read because they can adequately explain their evidence, but aren't sticking to memorizing facts.
