In the “Area of Study” column write the course, subject, topic, or work student studied. Any other information, such as a textbook title or online course number may also be written here.
In the “Terms” columns, write the score, the grade, the grade level currently working on, or the grade level completed. Choose what is most appropriate for your family and child. You may choose to indicate what information you are writing in each “Term” column.

If a child only takes a course for a term or two, indicate this by coloring in the term boxes the course was not taken. You can also indicate this by writing a dash or “x” in the box.
Consider using colored inks or pencils to indicate different terms for an easy visualization.
This form may be printed on duplicate or triplicate paper so a copy can be given to the proper authorities and copies can be retained at home.
