My grade level assigns some kind of review homework most nights, but I also like to incorporate games, reading or online practice one or more nights a week. I encourage my students to read EVERY night, but I know some don't read at night unless it is specifically assigned.
I created this simple log as a way to hold students accountable. I encourage them to read with an adult, a sibling, a pet, stuffed animals, in a mirror, or over the phone with a friend. All of this encourages better fluency!
This log can be copied and cut in half to save paper. I review the slip before sending it home and explain how to fill it out. Multiple boxes can be checked if more than one choice applies to their reading. I think this paper works best with primary students (because they may be reading a shorter book in one evening), but my intermediate students enjoy it, too! After reviewing the slip, we go ahead and write our names on the slip and slide it into a book they are currently reading before packing in their backpacks.
I don't take a grade over these slips, but I like to respond to my students once they share their thoughts on their reading.
