This is the homework template I use in my classroom and is a free download. I change it each week and it has room for quick notes to parents in the header. It is also easy to change up when differentiating homework assignments for students.
I found that it was more student friendly to give the Spanish version for families to keep as a reference at home. My students preferred the English copy, but their families definitely used the Spanish copy as they helped their children with homework. You will know what works best for you and your students.
My students fill in (dot the color-green, yellow or red) their AM behavior before lunch and then their PM behavior at the end of the day. If student fills in yellow, I write a quick note explaining the change in color and red would be a phone call home.
Since I do not post a behavior chart on the wall, I save this copy and put it in each student's file on Friday as it serves many purposes. It is a document of parent-teacher communication, homework assignments, a visual for behavioral concerns and subsequent notes (positive and negative) included about behavior.
