This can be used for when a child does not do well on a test. It includes the a place for the child's name, homeroom, date, type of test, and grade made. The child then has to write an explanation of why they did poorly on the test and also an explanation of what they will do next time in order to do better. This then has to be signed by the child, teacher, and parent in order for all to be held accountable to helping the child do better next time. This is also great for holding the student accountable for his work. The form is an editable version. I use this in my classroom all the time and it works great! It's also wonderful for when you have parent conferences, and the parent swears you've never told him his child was doing poorly. You can have this as proof that the parent has seen it by showing his signature!
