This chart is used for students that require breaks throughout the school day. This can be used to support a child's IEP program modifications or accomodations.
Simply cut the pieces, mount on construction paper or cardstock, and laminate for extended use. I used Velcro dots on the chart and pieces so the students can rip break cards off at the end of each day.
How I Use This: I give the child three break cards (with the clocks). The child stores the break cards in their pencil box and presents one to me when they want/need a break. When they are given permission, they stick the break card to the chart pictured. After three breaks, they can no longer ask for a break. Depending on the student, you may grant them three breaks in the AM and three in the afternoon. Some students can manage to only take three breaks in an entire day.
