It's always around this time of the year that my students supplies run out, get broken, or seem to have gotten up and wandered away! I find myself quickly scribbling down on a post-it or notecard the supplies they need and just hope the little note finds it's way home. But now with this worksheet, I've got a cute, simple way to ask parents to send in more supplies. The note reads: "I've been working hard in school, each and everyday. But I've run out of these supplies, would you please send some my way?" Then, just circle what they need to bring it and send it home!
Teacher tip: I keep a stack of these already copied by my mailboxes. That way, when a student tells me they ran out/lost/broke their supply, I just grab a worksheet, fill it out, and pop it into their mailbox!
