These charts are organized so that you can see at a glance what accommodations each child requires and which children require the same specific accommodation. I have included sample student names and sample accommodations for each subject so you can see how it works. I’ve left the whole package as a Power Point so you may adapt it to your district/school/students’ needs. Simply type into the boxes as needed.
Download and save this document. As you receive IEP information, delete my pretend names and insert your student’s name and accommodations into the appropriate page(s). If you want a hard copy for quick reference and portability to meetings (I always do), print each page and slip each one into a protective sleeve. Combine everything into a highly confidential three-ring binder.
