At the beginning of the year I make data binders for each student. Inside their binder I keep an IEP goal page for each of their IEP goals. I put their goals in kid friendly terms. We start a graph and the students continue their graph throughout the year. It helps the students take ownership of their progress. I've found that informing them of their progress and their goals also helps them motivate themselves to improve.
In this download, you will find two examples of what my data sheets look like. I included three different templates that you can use for your own students. One has a blank graph so that you can make your own labels. One is set up for percents up the left side and has months of the year along the bottom. The last template has only the months of the year. Feel free to print them out and alter them to fit your needs.
I've found it incredibly useful to include a target for my students. It helps them see where they need to be. When they're coloring in their graphs they can see if they are above target and working toward their goal, or if they're below target and need additional support. It really helps invest the students in their own progress.
