This is a Google Forms survey that gives a general idea of a how much your students know about their IEP. I developed this as a Special Day Program teacher, and it has informed my practice greatly. Some people may be uncomfortable at first with discussing IEPs with students, but communication is critical! Having these conversations with parents and students is imperative, especially at the middle school level. Middle school students in the SDC environment are more than aware that their classes are different than others on campus. Students in co-teaching or RSP models notice the meetings, testing, accommodations, learning centers, etc. that make them different too! This survey can help you collect useful data as you plan for self-advocacy, self-determination, or empowerment lessons. You could even use this at the beginning of the year to assess how students feel about having an IEP so you can address misconceptions, negative stereotypes, and poor self image. It can also be an effective pre/post test to measure IEP knowledge and awareness when involving students in the IEP meeting process. I've included a PDF version in case you don't have access to technology or you/the student simply prefer to work on paper.
