This is the basic format of the data collection sheet I've used for years to collect my student's progress towards their IEP goals. In the header of the document I type the student's name. In the far left column of the chart, I type out all of the student's IEP goals/objectives. Then the columns to the right are used for data collection each week. I put text in the box to match the goal to make data collection quick and easy (letters listed for letter identification objective, etc.). When I am assessing the student, I mark the letters missed and correct, making instruction and progress reports super quick and easy!
To make things even easier, keep an area of data collection tools (various flashcards, clocks, reading passages, counting manipulatives, etc.) in your classroom that are easily accessed!
