I have used this chart at the beginning of the year. It is easily filled in by me, the special education teacher, and distributed to all relevant staff members (general education teachers, specialists, and administration). It allows the staff to get basic information about each student in a grade level, who has an IEP on one piece of paper. For example, my Librarian likes getting this information because if a 5th grade student is reading at a first grade level, she will have to accommodate for this student during Library instruction. This chart has a "Joe" student as an example, which can be deleted upon purchasing.
