Keep efficient, weekly track of your student's progress towards their reading IEP goal!

In this packet you will find:
- 11 separate PDF files, one for each month of the school year. These files are divided into trimesters according to IEP dates and give specific indicators as to when each short term objective should be met, and when to move on to the next. This helps keep yourself and your staff on track. There are also indicators as to when to start preparing for the next IEP.

I use these generic data sheets for each of my student's individual reading goals. The check points make it easy to see their progress at a glance. The "activity" portion allows you to show that the activities you are doing align with their IEP goal(s). The comment section allows for elaborating on specific skills they are working on, and detail what areas they need further instruction in. The prompt level shows where they are at in moving towards independence. This is a crucial element as the student may be making progress on their goal, but still require prompting from an adult to complete the task at hand.

Each section only takes a couple minutes per student to fill out and it allows for a comprehensive look at their progress throughout the year.
Each data table is split up according to the "quarters" of the student's IEP. As one quarter ends, I make a note to show that their short-term objective one should be met, and it is time to move on to short-term objective two, their annual goal etc. This helps keep myself and my staff (Instructional Assistants/Para Educators) on track as to exactly where the child should be in regards to their goal.

I have data sheets such as this for each student, for each goal area. (For other goal areas, see my large IEP Data sheets packet which includes reading, writing, math and behavior goal areas.) I have my IAs fill out the weekly data for the groups that they run. My students are in 3rd through 6th grade and working on academic skills ranging from a pre-acadmeic level to a third grade level. With one unique exception, I use this "generic" form for all students, regardless of level. I keep the forms in the student's subject specific folder that also holds their work.
