This is a free resource PNG download created by a Masters in Ed. student Miguel G. and given permission to share as a free resource. Miguel said "The template includes room for information such as accommodations, services received (with dates and time), strengths, and goals for the student. It also includes a section for additional information that we should know about the student. ...The goal for this template is for every member of the IEP team to receive a copy..... With the template, my hope is that it can be filled with information that will help us better understand the student that we are working with. One last thing I would have included would be the case manager contact information in case the parent of a student has a question or concern regarding the IEP."
