As special education teachers we need to communicate the needs of our students to their general education teachers and paraprofessionals. Gen. Ed. Teachers and paras need to know which accommodations and modifications to provide to students, when to apply accommodations, and for how long. They also need to know the strengths and areas of concern to adequately address their deficits. This form helps to do just that.
Let teachers and paras know a student's related services, exceptionality, IEP due date, behavior plan status, goals and any other information you feel they need to know to help that student be successful. All of this can conveniently be accomplished in one form and then attached to an email for each teacher/para working with students on your caseload.
