This document not only consolidates a student's IEP and program/personal goal into one document, but it also helps the students take ownership and track their own goals (great for Danielson Model evals!) I find that a lot of my students have no idea what their IEP goals are, and I wanted to change that. With my students, I printed their "IEP At A Glance" and had them input their goal and time frame into the document. A couple of weeks prior to the quarter ending, we meet to discuss their progress in order to focus on our work. Then quarterly, we meet to discuss/determine if they met their goal or benchmark. The students lead the discussion and make the determination; I simply give input and help facilitate.
