These forms are perfect for the regular education inclusion teacher. We all know documentation is key when it comes to servicing children with special needs.
The first page in this document allows you to keep track of all students in each grade level who are on an IEP or a 504. (I fill out one of these for each grade level I teach.) This alleviates the need to flip through mounds of paper work and is great for a quick glance as to whether or not you are fulfilling the needs of all of these students.
The second page allows you to document all accommodations and modifications you make for each individual student. This comes in handy during parent-teacher conferences, IEP meetings, etc.
