I use this for myself, my students, and my staff in order to have a organized and clear place for all the most important parts of the student's IEP. My transition-aged student's use this to keep track of their own IEP goals, look at their accommodations and modifications, and remind themselves of the vision they created. Additionally, there is space for behavior strategies, reminders, and student personal goals, a place for notes, and a communications log.
I use this for my own caseload binder, give a version without the communication's log and certain personal information to my staff for each student, and give each student a copy for their own binders. I type some notes for behavior and strategies from the beginning, but print with room for staff and students to add as things change and as we have meetings and progress is made.
This is made with a transition aged Massachusetts student in mind, but is editable for use in other states or with younger students.
