I use this form all the time as a high school special education teacher, but it is also very handy for general education teachers. We all know it is never a quick look into an IEP and when you are in the middle of a small group class or a general education class with 30 kids, it is necessary to have your information organized and handy.
I have included contact information for case manager and parents. Important IEP information: dates, critical health info, medications, glasses, services (when/where), and accommodations in classroom and testing.
Special Ed teachers: I keep a copy for all my caseload students in their working files. I have a binder for each of my small group classes and any other students I support in a mainstream class for quick reference to assure accommodations and service minutes are being met. It can also be helpful to share this form with your students' other teachers.
General Ed teachers: I have taught in a general ed classroom as well and have used this form to help me quick reference accommodations for testing, parent/case manager contact information or when students need to take medications. It can be a great tool to have and use!
