I made up a new version of a form I had been using the past few months to help me out with annual and initial IEPs for preschool students. Have you ever needed the teacher's input for IEPs but never knew exactly the topics to cover for a preschooler? Or, forget to ask the teacher strengths and weaknesses? I know I have!
This IEP teacher summary report has helped me these past few months:
•I have used it as a guide for when I interviewed the teacher when I needed information for my speech and language assessment reports (especially with the communication section).
•Sometimes, I would end up having the teacher complete this form as well for the actual IEP meeting.
• I like to pre fill out the information on the present levels page prior to the meeting and tweak each section as the meeting is occurring so that the most updated information is on the IEP.
