2) A guided worksheet AND exit slip assessment for teaching students how to convert improper fractions into mixed numbers using fraction rod manipulatives (http://www.hand2mind.com/item/rainbow-fractiondecimal-tiles-set/1187). If you do not own rods, the Super Teacher website offers paper fraction manipulatives which can be laminated and cut.
3) An "additional support" template for students who are struggling with converting improper fractions into mixed numbers. You would need to add the examples, but the steps and "check" are provided.
4) A guided worksheet AND exit ticket assessment for teaching students how to convert mixed numbers into improper fractions. There are no manipulatives involved, it is just teaching them the method; although you can absolutely include manipulatives as you see fit.
