This form allows you to track incidents involving a behaviour of concern.
Everyone involved with the child/student at school, home, outside of school and home, needs to record when an incident happens involving a specific behaviour. This form lets you track when, where and what was happening before, during and after the incident as well as how each incident is dealt with. Tracking should be done over a week or two.
Once you have this information you need to use the and Brainstorming sheet to work out triggers for the behaviour, i.e. what is really causing it, how it is dealt with, what is the best way it is dealt with and what could be done differently to deal with it. You can find out what skills need to be taught or what resources are needed to help teach the individual other ways of dealing with situations, what replacement activities could be used or what is needed to teach social/emotional/communication skills the child/student lacks or doesn't understand.
