After I teach my minilesson for Reader's Workshop (RWS), I send the students off to read and work independently. All my readers know that the goal is for them to try something from that day's minilesson. To simply try. After they attempt, they can move on to something else they're working on as a reader, a skill or strategy from a previous lesson and such. This sheet guides them on recording what they tried from the minilesson that day. It keeps them and myself accountable for what's going on during workshop as I am not able to confer with every single student.
