This is a generic, blank independent reading book conference or recommendation form that can be customized for any class. I use this with my middle and high school students instead of a traditional book report. The form has space for students to rate the book, list its genre, the author, title, number of pages and then provide a short synopsis of the plot. Following this, students answer questions about the setting, point of view, protagonist, antagonist, conflict, climax, theme, and literary terms. Finally, students give their recommendation of the book and state which book they plan on reading next. Students follow up by bringing this form to me for a book conference.
This is a Microsoft Word document and can be modified to fit your particular classroom or application. The header currently says "Lang. Arts 8" as the class, but this is only an example and can be quickly and easily changed. Produced by Suzann Keith.
