This is a form to track students independent reading. Keep a binder for independent reading and a whole page for each child. Write down the book the child is reading and the page they are on. This only takes a few minutes each day and allows you to keep track of independent reading, trends, favorite genres, etc. Record dates independent reading conference are held. There is also a spot to record the score if the child takes a test on the book. Once a student has finished their book, highlight the box and move to the next box. Record pages numbers daily. I required my third graders to read 10 pages a night. This helped me keep track of their independent reading. Research supports number of pages not minutes.
