Inspired by The Book Whisperer I wanted to make independent reading a major component of my instruction. I use these forms to document independent reading conferences. The students fill out the top boxes in their form as I fill in the matching boxes in the teacher form. Then we set goals together. The students keep their copy in their reading folder. The teacher form has extra boxes to add notes about their reading comprehension, fluency and metacognition as observed in the conference. Each student is assigned a day of the week for our conference so they know when to expect to meet with me. There is a box on the form to keep track of how many reading responses (RR) they have completed. I have included the reading conference schedule form as well.
