Do you notice that your students are reading, but not completing books? This is something that I noticed. I created a program to help hold students accountable for completing books.
Each student sets a monthly goal on how many books they are planning on completing. Students keep track of their goals on 2 separate data sheets (monthly and one for the school year) that can be added to their data folders, if they are keeping them. Once they finish a book, they fill out a bookmark. On the bookmark is a space for a summary and a book review. Hole punch the bookmarks and keep them on a ring so students can easily access them all year long. At the end of the month, students will fill out a reflection sheet about their goal.

Not included: Classroom chart to hang on a data wall. Any type of poster sized incentive chart will work.
