Use this data sheet to document what percentage of time your student is on-task during independent work periods.
The sheet is designed to be used with a momentary time sampling procedure, as follows:
1. Provide the student with a task that can be completed independently.
2. Record the date, time of day, and activity at the top of each data block.
3. Using a timer or watch with a second hand, observe the student at the end of each one-minute interval.
4. Mark a + if the student is demonstrating independent, on-task behavior at the end of the interval, or a - if the student is engaged in off-task behavior or receiving a prompt from another person at the time of the sample.
