Directions: Fill in activity based on daily schedule (adjust times as needed). Mark a tally each time a student asks for individual assistance. Request for individual assistance could look like any of the following: check in, ask to repeat directions, assistance in solving a problem. One sheet should be used per student. It is important to also describe the activity the student is specifically working on and the setting (e.g., math worksheet in small group, independent reading)
