This is a resource for school counselors who meet with students for individual counseling sessions. (Those students who come regularly.) I keep these sheets on a clipboard and grab one when I begin initial meetings with students. Once I start a form for a student it goes into a binder divided by grade level. I can grab the binder and add to my notes as students continue to visit.
Before this I used to use random pieces of paper that would float around everywhere in a mess! This has been much more helpful - to have certain forms to fill out and a special binder and clipboard to store them. I know that school counselors and note taking can be a topic of discussion, but if I don't write at least something down I'm sure to forget with my jam packed days!
It includes a cover sheet (in color) for your clipboard, a cover sheet (in color) for your binder, and the main notes page. Also includes a version with a girl clipart character and a version with a boy clipart character.
