This form gives you an easy way to let others know some specifics about a child with behavior issues. This form breaks down what all stages of the crisis scale look like on the students and ways to reward or deescalate the situation. I found that this is very helpful for general education teachers and even bus drivers as a guide of what not to do in certain situations with children with behavior or emotional disorders.
I would recommend giving this to any teacher who will be interact with children with behavior issues. Have them sign that they have received a copy, much like you would with IEP documents. Then, if there is an issue, you can refer back to the document and see if anything is not being followed.
