Use the first sheet to record your students' individual learning goals. Record the date you set the goal, the three dates on which you observed evidence of the goal and the date you and your student decided the goal had been achieved.
The second page is for you to write the goal for your student and then mark off the three pieces of evidence that you observe. This slip can be sent home to show what great progress your student is making!
I have my goals attached to silver cardboard and stuck to my students' tables for quick reference.
