This is a behavior contract designed for a secondary school classroom that runs on a block schedule (teachers see students three times a week). I have had huge success using this with key students that need an adult to believe that they can exhibit positive behaviors. The students work with the teacher to set their own weekly goal. The teacher elicits from the student three ways they can reach their goal. Each day, the student gets a score for the three behaviors. At the end of the week, the scores are added up, and if they are within a specific range of scores as deemed appropriate by the teacher, the student gets to select one of the rewards listed at the bottom. Students take home the contract on Thursdays to be returned signed by a parent on Friday. This can be altered to fit your specific classroom needs.
