This is a useful form for tracking and replacing textbooks and library books.
Our district's processes and forms for book replacement was too long and complicated for parents to complete. Fortunately, our site was given permission to simplify the process.
This is a convenient form that holds all of the essential information. Once a book needs to be replaced, simply make a copy of the original agreement, attach it to the replacement form and send it home. The form simply and clearly notifies parents of the situation and the cost(s) involved.
