The Individual Student Plan of Action is designed to meet the needs of your student. It is intended for the parent and teacher to work together to keep the student on track. It can be distributed every week, bi-weekly, or monthly. It indicates the special attention the student receives in the classroom throughout the week, as well as, help at home. It also includes strengths, weaknesses, and goals for the student. You may choose to only utilize it for struggling students or you may choose to do it for each child in your classroom. In my classroom I send it home every other week for each child. The parents really like seeing exactly how instruction is individualized for their child.
